Sundays and national holidays, this clinic is off.

When you come for the examination of yourself, you are requested to bring the following certificates, documents or things.

Health insurance card issued by Japanese government.
Examination card issued by this clinic.
If you are taking any medicine prescribed by other hospitals, or if you were taking any medicine when you were in your home country, show us the prescriptions
or descriptions of the medicine you are/were taking (this is to prevent adverse effect which might be caused by administering our medicine if it conflicts
with the medicine you are/were taking).

When you come for the examination of your child, you are requested to bring the following certificates, documents or things.

Mother-child health handbook ("Boshi Techo" in Japanese) issued by Saitama city (if you have one issued by your country, bring it and show it to us too).
Examination card issued by this clinic.
Medical check sheet for vaccination issued by Saitama city ("Yoshin Hyo" in Japanese) (please write the body temperature of the child before you come).

If your child is very young, i.e. an infant, bring the following certificates, documents or things.
Note that there are 4 types of infant as follows. Make sure which type your child belongs to.

Medical chack for 4 month old infants	From 4 months up to the age younger than 6 months old
Medical check for 10 month old infants	From 10 months up to the younger than 12 months old
Medical check for one and a half year old infants	From one and a half years up to the age younger than 2 years old
Medical check for 3 year old infants	From 3 and a half years up to the age younger than 4 years old

Mother-child health handbook (see above).
Examination card issued by this clinic.
Medical check sheet for examination issued by Saitama city. A different check sheet is issued to each infant type.
Bath towel.
Urine of the child taken in the morning of the day you come (only applicable to a child who is 3 years old).
